The "Hot Cheetos send people to the ER" headline is mostly a meme. The actual chemistry is a lot less dramatic — and a lot funnier — than the panic suggests.

Two separate things are happening

One: the snack is dusted in capsaicin and a punchy seasoning blend. That heat survives the trip through your gut largely intact. Two: the red dye that gives the snack its glow does not always get absorbed, so it can pass through and stain the result. The burn and the color are unrelated. They are just sharing a ride.

Why it stings on the way out

Your gut has the same TRPV1 heat receptors your mouth does. Capsaicin activates them whether it is going in or coming out. Add the friction of a snack that is essentially crispy seasoning and there is not much mystery about why it bites.

The ER myth

You may have seen the headlines about kids being rushed to hospitals after a bag of Flamin' Hots. Most of those visits turn out to be scared parents seeing red and assuming the worst. Actual gastritis from snacks is rare. The visit is almost always "false alarm, hydrate, eat something bland." That said, real symptoms exist — see the last section. Red on toilet paper that is clearly blood — not snack dye — is worth a call. So is burning that lasts more than a few days, severe abdominal pain, fever, dizziness, or repeated vomiting. Snacks should not cause any of those.
